- High-grade B-cell lymphoma, NOS (HGBCL, NOS) is defined morphologically between Burkitt lymphoma and diffuse large B-cell lymphoma (DLBCL), excluding cases with specific genetic aberrations.
- An LLMPP study involving expert hematopathologists reviewed 92 cases submitted as HGBCL, NOS, showing poor interobserver agreement.
- Only 39 out of 92 (42%) cases were confirmed as HGBCL, NOS after independent and consensus reviews, with many reclassified as DLBCL or other aggressive B-cell lymphomas.
- No significant differences were found between initially submitted and confirmed cases regarding dark zone signature expression, MYC rearrangements, or cell of origin.
- The study suggests HGBCL, NOS may not represent a distinct clinicopathologic entity, and more objective markers like dark zone signature could better define diagnostic categories than morphology alone.
