- The study investigated the effects of finerenone, a nonsteroidal mineralocorticoid receptor antagonist, in adults with chronic kidney disease (CKD) who did not have diabetes.
- A total of 1,584 participants with CKD (eGFR 25 to <90 ml/min/1.73 m² and albuminuria) on renin-angiotensin system inhibitors were randomized to finerenone or placebo.
- The primary outcome, the total eGFR slope over 32 months, showed less decline with finerenone (-3.3 ml/min/1.73 m² per year) versus placebo (-4.0 ml/min/1.73 m² per year), with a significant difference of 0.7 ml/min/1.73 m² per year (P<0.001).
- Finerenone reduced the risk of a composite kidney or cardiovascular outcome event by 23% (hazard ratio 0.77, 95% CI 0.60 to 0.99, P=0.04).
- Hyperkalemia was a common adverse event, occurring more often with finerenone (17.0%) than placebo (13.3%), leading to discontinuation in some participants.
