- Rare case of Mycobacterium avium complex (MAC) pulmonary disease in a 78-year-old male with rheumatoid arthritis (RA) and interstitial lung disease (ILD) under non-biologic therapy.
- Patient presented with progressive dyspnea and chest tightness; high-resolution CT showed asymmetric ILD progression with tree-in-bud signs and nodules, confirmed via bronchoscopy and metagenomic sequencing.
- Treated with a 4-drug anti-MAC regimen but lost to follow-up and died, highlighting need for proactive atypical pathogen screening in RA-ILD patients even without glucocorticoid or biologic exposure.
