- Hodgkin lymphoma (HL) is a B-cell malignancy often affecting young adults, with risk stratification based on PET/CT staging and presence of risk factors.
- For early-stage favorable classic HL, standard treatment is two cycles of ABVD followed by 20 Gy involved-site radiotherapy.
- For early-stage unfavorable classic HL in patients ≤60 years, consider two cycles of eBEACOPP (or procarbazine-free variant) plus two cycles of ABVD, with PET/CT-guided radiotherapy.
- For advanced classic HL in patients ≤60 years, consider BrECADD for 4-6 cycles (based on PET/CT response) followed by PET/CT-guided radiotherapy.
- For older patients (>60 years) or those preferring less intensive approaches, options include 4 cycles of A(B)VD with radiotherapy or 6 cycles of N-AVD with radiotherapy.
- Relapsed classic HL should be treated with checkpoint inhibitor salvage therapy followed by high-dose chemotherapy and autologous stem cell transplant if eligible.
- Nodular lymphocyte-predominant HL may differ in treatment, potentially including anti-CD20 antibodies.
- The guideline provides recommendations for diagnosis, staging, treatment, and follow-up of Hodgkin lymphoma.
