- This is the first reported case of a Streptococcus intermedius cerebellar abscess as the initial presentation of sarcoidosis.
- A 45-year-old male with a history of liver cirrhosis presented with neurological symptoms; imaging showed a cerebellar abscess causing hydrocephalus.
- Emergency surgery (external ventricular drainage and craniectomy) was performed, with cultures confirming S. intermedius.
- Further workup revealed bilateral pulmonary infiltrates and bronchial biopsy showed non-caseating granulomas, leading to a diagnosis of sarcoidosis.
- The patient had T-cell lymphocytopenia, indicating an immunocompromised state, and received 6 weeks of targeted IV antibiotics.
- At 6-month follow-up, the patient was neurologically intact with complete abscess resolution on imaging.
