- Primary study objective was to compare safety and efficacy of emergency surgery (ES) versus self-expanding metal stents as a bridge to surgery (SEMS/BtS) for acute left-sided malignant colonic obstruction.
- Key finding: SEMS/BtS significantly reduced the 1-year stoma rate to 44.4% compared to 73.4% with ES, and lowered the risk of persistent stoma (adjusted OR 3.74 for ES).
- SEMS/BtS led to earlier initiation of chemotherapy (76.1% vs 55.8%) and fewer 30-day readmissions (4.2% vs 15.9%) without increasing short-term morbidity or mortality.
- Quality of life outcomes at one year were significantly better in the SEMS/BtS group, with comparable oncologic results, though follow-up duration was limited.
