  • The study investigated the association between Alzheimer's disease (AD) related pathological changes and cognitive impairment in amyotrophic lateral sclerosis (ALS).
  • Biomarkers including cerebrospinal fluid (CSF) total-tau, phosphorylated-tau, beta-amyloid, and plasma TDP-43 and neurofilament light chain were analyzed in 192 ALS or ALS-frontotemporal dementia patients and 100 healthy controls.
  • Patients with ALS, especially those with cognitive impairment (ALSci) or ALS-FTD, showed elevated AD-related biomarkers compared to healthy controls.
  • CSF p-tau and total-tau levels were strongly linked to domain-specific cognitive performance, while plasma TDP-43 and NfL showed weak or no association.
  • Postmortem data indicated that temporal and hippocampal TDP-43 burden, rather than overall TDP-43 or ABC scores, was associated with cognitive dysfunction.
  • The findings suggest tau-related CSF biomarkers are associated with cognitive deficits in ALS, but TDP-43 pathology may have an even stronger relationship postmortem.
