- Persistent Streptococcus pneumoniae vaccine serotypes were studied using 6,545 nasopharyngeal swabs from children hospitalized with pneumonia in Mongolia before and after PCV13 introduction.
- DNA microarray analysis showed changes in lineage composition within serotypes 6A, 6B, 14, 19F, and 23F over six years.
- Post-PCV lineages were associated with lower severity of pneumonia for serotypes 6B, 14, and 19F, and reduced multi-drug resistance for serotypes 6A, 6B, and 23F compared to pre-PCV lineages.
- The post-PCV lineage of serotype 6B exhibited lower antibody binding to capsule, reduced capsule gene expression, thinner capsule, and lower virulence in mice than the pre-PCV lineage.
- Vaccination may select for less virulent lineages within persistent vaccine serotypes, highlighting the importance of factors beyond serotype in evaluating vaccine effectiveness.
