- The POLAR trial is an international multicentre randomised controlled trial evaluating a dynamic PEEP strategy during resuscitation of extremely preterm infants at birth.
- It compares a dynamic PEEP starting at 8 cmH2O, titrated to response up to 12 cmH2O, against a static PEEP of 5-6 cmH2O.
- The primary outcome is death or bronchopulmonary dysplasia at 36 weeks' postmenstrual age.
- Secondary outcomes include short-term respiratory morbidity and potential harms in the first 10 days after birth.
- The trial plans to recruit 906 extremely preterm infants to assess the impact on neonatal care.
