- Finerenone, a nonsteroidal mineralocorticoid receptor antagonist, reduces the risk of sudden death in patients with cardio-kidney-metabolic (CKM) syndrome.
- The analysis pooled data from three trials (FIDELIO-DKD, FIGARO-DKD, FINEARTS-HF) involving 18,991 participants over a median follow-up of 2.9 years.
- Sudden death occurred in 418 participants (2.2%), with higher rates in heart failure trials compared to chronic kidney disease trials.
- Predictors of sudden death included older age, history of heart failure, atrial fibrillation, prior myocardial infarction, higher urine albumin-to-creatinine ratio, and lower baseline systolic blood pressure.
- Finerenone treatment resulted in a significant 19% relative risk reduction in sudden death compared to placebo (HR: 0.81; 95% CI: 0.67-0.98).
- Risk reductions were consistent across subgroups based on the number of baseline CKM conditions and trial type.
