- A female in her early 30s with a history of postpartum granulomatous hypophysitis presented with palpitations and syncope.
- She was hemodynamically unstable with polymorphic ventricular tachycardia (VT) and a prolonged QTc interval of 581 ms.
- Despite antiarrhythmic therapy, she experienced a VT storm, leading to cardiogenic shock and left ventricular dysfunction.
- Lab tests revealed severe central hypothyroidism with low T3 and T4 levels, excluding other causes of QTc prolongation.
- Treatment with thyroid hormone replacement (T3 and T4) and steroids resolved arrhythmias and restored ventricular function.
