  • Late electrical storm (ES), defined as ≥3 sustained ventricular arrhythmia episodes within 24 hours after 30 days post-implantation, occurred in 4.3% of LVAD recipients.
  • Patients with late ES had significantly higher 5-year all-cause mortality (aHR 2.87) and cardiac mortality (aHR 3.47) compared to those without.
  • Four independent predictors of late ES were identified: LVEDD ≥80 mm pre-implantation, non-ischemic cardiomyopathy, history of ventricular arrhythmias, and ICD before LVAD implantation.
  • A risk stratification score (Late ES-LVAD score) was developed, showing good discrimination (C-statistic 0.76) and stratifying patients into low, intermediate, and high-risk groups with increasing late ES rates.
  • Late ventricular arrhythmias without ES showed no prognostic impact, highlighting the specific risk associated with electrical storm events.
