- The study assessed the frequency and factors associated with herpes zoster (HZ) in Latin American SLE patients using data from the GLADEL 2.0 cohort.
- Among 1083 SLE patients, the cumulative incidence of HZ was 11.5%, with a prevalence of 8.6% at cohort entry. The incidence rate was 2.9% over 5 years, and 16.8% had recurrent episodes.
- Patients with HZ had higher frequencies of alopecia, psychosis, seizures, disease activity, damage accrual, proteinuria, and higher daily prednisone doses before the event.
- Multivariate analysis identified female sex, higher SLEDAI-2K scores, and higher daily prednisone doses as independent predictors of HZ occurrence.
- Older age at diagnosis, psychosis, disease activity, and higher daily prednisone doses were associated with a shorter time to HZ onset.
- The study highlights the high burden of HZ in SLE patients, linked to active disease, corticosteroid use, and neuropsychiatric manifestations, emphasizing the need for proactive risk stratification.
