  • MS participants showed higher mean qT1 values in ChP, normal-appearing white matter (NAWM), and gray matter (GM) compared to controls.
  • ChP enhancement (ΔT1) was significantly associated with pre-gadolinium qT1 values in NAWM and GM, but not with white matter lesion (WML) volume or qT1.
  • Findings suggest a CSF-mediated toxicity mechanism in MS brain injury, distinct from classic lesion pathology.
