Colitis-associated colorectal neoplasia in ulcerative colitis with primary sclerosing cholangitis: a nationwide study - PubMed

- Study examines colitis-associated colorectal neoplasia (UCAN) in ulcerative colitis (UC) patients with primary sclerosing cholangitis (PSC) in Japan.
- 808 UCAN patients (1983–2020) were categorized into PSC-UCAN (n=26) and UCAN-alone (n=782) groups.
- PSC-UCAN group had shorter UC duration before UCAN diagnosis (12.8 vs. 16.9 years) and younger age at diagnosis (47.8 vs. 53.3 years).
- PSC-UCAN group showed higher prevalence of right-sided colon UCAN (34.6% vs. 15.9%).
- PSC-UCAN group had lower high-grade dysplasia (19.2% vs. 30.7%) but higher early-stage cancers (53.9% vs. 31.2%).
- 10-year overall survival (64.6% vs. 79.3%) and cancer-specific survival (80.8% vs. 83.9%) were comparable between groups.
- Findings suggest PSC-UC patients benefit from early surveillance colonoscopy due to earlier and younger UCAN development.
