Comparing DSA-negative and DSA-positive antibody-mediated rejection in heart transplants: results from the Trifecta-Heart study - PubMed

  • The study compares DSA-negative and DSA-positive antibody-mediated rejection (ABMR) in heart transplants.
  • 212 heart endomyocardial biopsies (EMBs) from the Trifecta-Heart study were analyzed.
  • Molecular Microscope® Diagnostic System (MMDx) and histology were used to characterize rejection.
  • Donor-derived cell-free DNA (dd-cfDNA) and central plasma DSA were measured at the time of biopsy.
  • MMDx ABMR was 53% DSA-positive and 79% dd-cfDNA positive.
  • DSA-negative and DSA-positive molecular ABMR showed similar post-transplant days, %dd-cfDNA positivity, and mean LVEF.
  • DSA-negative ABMR had lower molecular ABMR activity (ABMRProb score 0.55 vs 0.70).
  • Gene expression profiles were nearly identical between DSA-negative and DSA-positive ABMR.
  • Both types of ABMR are associated with high plasma dd-cfDNA.
