- Study compares regional citrate anticoagulation (RCA) and systemic heparin anticoagulation (UFH) in patients on VA-ECMO and CRRT.
- Primary outcome: CRRT filter thrombosis; RCA showed lower risk compared to UFH (17.0% vs 26.6% at 72h).
- Median filter lifespan was similar between RCA (47 hours) and UFH (44 hours).
- RCA did not increase bleeding events (33.3% vs 37.6%) or severe metabolic complications.
- Conclusion: RCA is safe and effective, reducing thrombosis risk without additional complications in high-risk VA-ECMO patients.
