- Vasculitic neuropathy (VN) involves vessel inflammation damaging peripheral nerves, with features in mainland China understudied.
- A retrospective study of 112 VN patients showed axonal sensorimotor neuropathy, commonly affecting tibial (90.63%) and peroneal (87.50%) nerves.
- Systemic VN (SVN) had more frequent transmural inflammatory cell infiltration (ICI) than non-systemic VN (NSVN).
- Eosinophilic granulomatosis with polyangiitis (EGPA) was more common in VN without ICI, suggesting diverse vascular injury mechanisms.
- ICI-positive SVN patients exhibited more severe distal upper limb weakness and higher thrombosis rates.
- Treatment with glucocorticoids ± immunosuppressants led to remission in most patients, though 13 died from multi-organ complications (12 SVN, 1 NSVN).
- The 5-year survival rate was 80.23%, highlighting the importance of early diagnosis and treatment.
- Nerve biopsy remains the gold standard for VN diagnosis, with overall prognosis being relatively favorable.
