- Autoimmune cytopenias (AICs) can occur after alemtuzumab-induced renal transplant, including immune thrombocytopenia (ITP), autoimmune hemolytic anemia (AIHA), and Evans syndrome (ES).
- Incidence peaks at 18 and 36 months post-alemtuzumab, with a higher response rate to rituximab (91.6%) and sustained remission in some patients off thrombopoietin receptor agonists (TPO-RAs).
- Common adverse events are infections (62.5%), cardiovascular diseases (27.5%), and deep vein thrombosis (25%), necessitating antimicrobial and anticoagulant supportive measures.
