  • A 35-year-old farmer with fungal keratitis was successfully treated with therapeutic penetrating keratoplasty (TPK).
  • The patient had a corneal ulcer following an ocular injury during farm work and did not improve with initial antifungal treatments.
  • Diagnosis revealed Aspergillus lentulus, sensitive to voriconazole, but the condition worsened despite intensive therapy.
  • Emergency TPK was performed, including removal of infected tissue and irrigation with voriconazole.
  • Postoperative treatment included cyclosporine A instead of corticosteroids to preserve the graft.
  • Four months post-surgery, the patient was infection-free with improving vision.
  • TPK with anterior chamber lavage proved effective for fungal keratitis resistant to conventional treatments.
