- This retrospective real-world cohort study compared intensive intravenous (cyclophosphamide or rituximab) and less intensive oral (methotrexate, mycophenolate mofetil, or azathioprine) immunosuppressive treatments for idiopathic or immune-mediated retroperitoneal fibrosis.
- Overall treatment effectiveness, assessed via a composite clinical, laboratory, and radiological endpoint, did not significantly differ between groups, with 68% of evaluable patients achieving effectiveness (OR 2.67, 95% CI 0.41-17.17).
- Secondary outcomes, including renal function improvement, hydronephrosis resolution, periaortic mass thickness reduction, relapse, and safety, also showed no significant differences between treatment strategies, supporting an individualized approach based on disease severity and patient factors.
