Efficacy and Safety of Newer Antibiotics Versus Generic Antibiotics for Hospital-Acquired and Ventilator-Associated Bacterial Pneumonia: A Systematic Review and Meta-analysis of Randomized Controlled

This systematic review and meta-analysis compares newer antibiotics versus generic antibiotics for hospital-acquired (HABP) and ventilator-associated bacterial pneumonia (VABP) based on 8 randomized controlled trials (RCTs) from 2013-2025 involving 2881 patients.
No significant differences were found between newer and generic antibiotics for the primary outcome of 28-day all-cause mortality or for secondary outcomes including clinical and microbiological response.
Newer antibiotics significantly reduced the risk of nephrotoxicity compared to generic regimens containing colistin, with a risk ratio of 0.30.
In subgroup analysis, newer antibiotics showed significant improvement in microbiological eradication for carbapenem-resistant gram-negative bacilli.
The study concludes that newer and generic antibiotics have similar efficacy and safety for HABP/VABP, but suggests future trials should target patients with carbapenem-resistant infections to further explore therapeutic benefits.
