- The ketogenic diet (KD) led to a significantly greater reduction in hepatic fat accumulation (CAP) compared to calorie-restricted diet (CRD) in patients with metabolic dysfunction-associated steatotic liver disease (MASLD).
- KD resulted in a higher proportion of patients achieving clinically meaningful reductions in steatosis and showed more significant improvements in body composition parameters like BMI, fat mass, and visceral fat grade, but also caused greater muscle mass loss.
- Only fasting insulin levels improved significantly more with KD versus CRD among blood biomarkers, with no major differences in liver enzymes, lipids, glucose, or inflammatory markers; both diets similarly reduced liver stiffness.
