  • Neuropsychiatric symptoms (NPS) in Alzheimer's disease (AD) were studied using the Neuropsychiatric Inventory Questionnaire (NPI-Q).
  • Four symptom clusters were identified: hyperactivity, psychosis, neurovegetative, and affective, explaining 62% of variance.
  • Hyperactivity cluster was associated with left middle temporal and right nucleus accumbens volumes.
  • Neurovegetative cluster showed associations with left middle temporal and right nucleus accumbens volumes.
  • Affective cluster had strong associations with left rostral anterior cingulate and right medial orbitofrontal cortex volumes.
  • All clusters predicted instrumental activities of daily living (iADL) outcomes; hyperactivity, neurovegetative, and affective clusters also predicted ADL outcomes.
  • Greater NPS burden, male sex, age, lower cognitive scores, and later visits predicted worse functional outcomes.
  • The study supports a cluster-based approach linking behavioral symptoms with brain structure and functional decline in AD.
