  • P-wave peak time (PWPT) is a novel electrocardiographic marker linked to atrial conduction delay and structural remodeling.
  • The study investigated the relationship between PWPT and albuminuria in hypertensive patients.
  • 367 hypertensive patients were categorized into albuminuria-positive (UACR > 30 mg/g) and albuminuria-negative (UACR < 30 mg/g) groups.
  • PWPT was significantly prolonged in patients with albuminuria (48 ± 11 ms vs. 39 ± 9 ms, p < 0.001).
  • Multivariate analysis showed PWPT as an independent predictor of albuminuria (OR = 2.688, 95% CI: 1.969-3.672, p < 0.001).
  • PWPT exhibited the highest discriminatory power (AUC = 0.771) with a cutoff of >42 ms (76.4% sensitivity, 68.1% specificity).
  • Conclusion: PWPT may serve as a noninvasive marker of early renal microvascular involvement in hypertensive patients.
