- Upper extremity deep vein thrombosis (UEDVT) is common in cancer patients, with limited data on recurrence and bleeding risks.
- The study analyzed 5,195 UEDVT patients from the RIETE registry, including 2,210 with cancer, over 24 months.
- Cancer patients had higher cumulative incidences of recurrent VTE (3.9% vs. 2.5%) and bleeding (6.8% vs. 4.3%) compared to non-cancer patients.
- Recurrence rates varied by cancer site: highest in lung cancer (6.4%), followed by gastrointestinal (4.0%), genitourinary (3.6%), breast (2.5%), and hematologic (1.4%).
- During anticoagulation, lung cancer and younger age were associated with recurrence, while genitourinary cancer and transient risk factors were linked to bleeding.
- After anticoagulation discontinuation, longer anticoagulation durations (91-180 days and >180 days) were associated with lower recurrence compared to 30 days or less.
- Cancer-associated UEDVT carries higher risks than noncancer UEDVT, with lung cancer having the highest recurrence during treatment and extended anticoagulation reducing post-treatment recurrence.
