  • MD significantly reduced procoagulant and prothrombotic microvesicles compared to LFD.
  • MD showed greater reductions in total cholesterol, cholesterol-HDL ratios, insulin levels, blood glucose, leukocyte count, and BMI.
  • MD is more effective than LFD in improving CV risk factors and outcomes.
  • The findings support recommending MD over LFD for secondary CVD prevention.
  • Further research is needed on long-term adherence and implementation in diverse populations.
