  • The study compares transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) in patients with severe aortic stenosis and pre-existing permanent pacemakers (PPM) or implantable cardioverter-defibrillators (ICDs).
  • Using the National Inpatient Sample from 2016-2022, 8776 hospitalizations were identified, with 501 propensity-matched pairs created.
  • TAVR showed significantly lower in-hospital mortality compared to SAVR (0.80% vs 2.59%), with a matched odds ratio of 0.31.
  • TAVR was associated with reduced rates of major adverse cardiovascular and cerebrovascular events, stroke/transient ischemic attack, acute kidney injury, major bleeding/transfusion, cardiogenic shock, respiratory failure, and lead revision.
  • Patients undergoing TAVR had shorter hospital stays and were less likely to have nonroutine discharges, without an increase in new pacemaker procedures.
  • The findings suggest TAVR is a favorable treatment option for selected patients with pre-existing cardiac rhythm devices.
