- The 2025 ATS guidelines support lung ultrasound for CAP diagnosis when performed by experienced clinicians.
- For healthy patients with CAP and positive viral tests, antibiotics are not recommended based on low-quality evidence, though imaging findings should be considered.
- Antibiotics are advised for patients with comorbidities, those admitted, or with imaging consistent with pneumonia.
- Antibiotic duration should be less than 5 days (minimum 3 days) for outpatients and admitted nonsevere CAP patients who achieve clinical stability, instead of ≥5 days.
- For severe CAP in admitted patients, antibiotic therapy should be 5 or more days.
- Corticosteroids may be used in severe CAP but are not recommended for nonsevere pneumonia.
