- Plasma p-tau217 measured by the Elecsys automated immunoassay shows good diagnostic performance for Alzheimer's disease (AD).
- In a cohort of 303 patients with mild cognitive impairment or mild dementia, plasma p-tau217 outperformed plasma p-tau181 with an AUC of 0.93 vs 0.87.
- Two thresholds were established: upper cutoff >0.312 pg/mL (95% specificity) and lower cutoff <0.177 pg/mL (95% sensitivity), with 27% of cases in an indeterminate range.
- Plasma p-tau217 strongly correlated with CSF Aβ42/Aβ40 and p-tau181/Aβ42 ratios, and plasma GFAP, indicating links to AD pathology and neuroinflammation.
- Moderate correlations were found with CSF t-tau, p-tau181, and plasma NfL, supporting its utility as a blood-based biomarker in heterogeneous memory clinic populations.
