  • The study developed a novel sdLDL-C-glucose (sdLG) index to predict cardiovascular disease (CVD) risk in middle-aged and older adults.
  • The sdLG index outperformed the traditional triglyceride-glucose (TyG) index in long-term discrimination for incident CVD.
  • When combined with the Chinese visceral adiposity index (cVAI), the resulting sdLG-cVAI showed the best predictive performance among derived indices.
  • Higher baseline and cumulative sdLG-cVAI levels were linearly associated with increased CVD risk, with HRs of 1.77 and 1.87 for the highest quartiles, respectively.
  • A persistently high trajectory of sdLG-cVAI was linked to a 75% higher CVD risk compared to a stable low trajectory, and results were robust in sensitivity analyses.
