- The study examines the bidirectional relationship between worsening kidney function (WKF) and cardiovascular events in type 2 diabetes (T2D) patients with atherosclerotic cardiovascular disease (ASCVD).
- Analysis of 14,671 patients from the TECOS trial showed that WKF, defined as a ≥40% decline in glomerular filtration rate or initiation of renal replacement therapy, occurred in 7.0% over a median 3-year follow-up.
- Nonfatal cardiovascular events (myocardial infarction, stroke, unstable angina) increased the risk of WKF (adjusted HR 2.0), with an even higher risk after hospitalization for heart failure (adjusted HR 6.1).
- WKF was associated with increased risks of major adverse cardiovascular events (adjusted HR 1.9), hospitalization for heart failure (adjusted HR 3.2), and severe hypoglycemia (adjusted HR 2.8).
- The findings highlight the need for early cardiorenal therapies in T2D patients with ASCVD to manage interconnected kidney and cardiovascular risks.
