  • Polygenic risk scores (PRSs) show potential for improving risk stratification for pancreatic ductal adenocarcinoma (PDAC), but existing models differ in design and performance.
  • This study developed genome-wide PRSs using methods like LDpred2 and PRS-CS, and evaluated them alongside 13 published models across diverse ancestry groups.
  • The genome-wide LDpred2-based PRS demonstrated the strongest association with PDAC risk, with an odds ratio of 1.57 per standard deviation increase, and significantly improved discrimination beyond established risk factors.
  • The LDpred2 PRS showed consistent performance across African, Admixed American, and European ancestry populations, while other models were primarily effective only in European ancestry groups.
  • These findings support the use of genome-wide PRSs for multi-ancestry risk stratification and early detection strategies for PDAC.
