- A 51-year-old woman presented with massive proteinuria and normal creatinine levels.
- Diagnosed with concomitant immunoglobulin A nephropathy (IgAN) and membranous nephropathy (MN) with Fabry-like zebra bodies.
- Fabry-like zebra bodies, typically seen in Fabry disease, can also occur due to genetic disorders or drug-induced phospholipidosis.
- Treatment included angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers.
- After 1.5 years, significant improvement was observed: creatinine decreased from 61 μmol/L to 42.9 μmol/L, and proteinuria reduced from 3.8 g/24 h to 52.5 mg/24 h.
- The case underscores the diagnostic and therapeutic challenges of co-occurring IgAN, MN, and Fabry-like zebra bodies, calling for more research on pathogenesis and treatment.
