Fosaprepitant versus Ondansetron for Preventing Postoperative Nausea and Vomiting After Video-Assisted Thoracoscopic Lung Resection: A Randomized Controlled Clinical Trial - PubMed

  • Fosaprepitant vs. ondansetron for preventing PONV after video-assisted thoracoscopic lung resection was studied.
  • 233 high-risk patients (Apfel score ≥ 2) were randomized to receive fosaprepitant 150 mg or ondansetron 8 mg, both with dexamethasone 5 mg.
  • Primary outcome: 24-hour PONV incidence (31.0% fosaprepitant vs. 41.0% ondansetron, P=0.112).
  • Fosaprepitant significantly reduced PONV at 48 hours (10.3% vs. 20.5%, P=0.032) and vomiting at 24 hours (8.6% vs. 21.4%, P=0.006).
  • Conclusion: Fosaprepitant was superior to ondansetron in preventing early vomiting and delayed PONV in high-risk thoracic surgery patients.
