Long-term treatment results of staged stereotactic radiosurgery for posterior fossa brain metastases with volume larger than 4 mL - PubMed

  • Staged stereotactic radiosurgery (SRS) is a safe and less invasive treatment option for selected patients with posterior fossa brain metastases (PFBMs) larger than 4 mL.
  • The study included 92 patients (96 PFBMs) treated with 24-30 Gy in 2 fractions, 3-4 weeks apart, resulting in a median volume reduction of 49% by the second session.
  • One- and two-year overall survival rates were 51% and 30% respectively, with local failure rates of 17% and 23%.
  • Rapid tumor response (≥50% volume reduction at second session) was associated with better local control (HR 0.32).
  • Karnofsky Performance Status scores significantly improved from 71 to 85 between sessions and to 90 at first follow-up.
  • Leptomeningeal metastasis rates remained low at 11% and 14% at one and two years.
