- Guillain-Barré syndrome (GBS) can have atypical relapsing presentations due to sustained immune dysregulation.
- A case report involves a 33-year-old man with motor-predominant acute inflammatory demyelinating polyradiculoneuropathy (AIDP), Graves' disease, and papillary thyroid carcinoma.
- Immunological findings showed B lymphocyte expansion, elevated interleukin-6 and interleukin-8, and thyroid autoantibodies, while anti-ganglioside antibodies were negative.
- The patient responded well to intravenous immunoglobulin (IVIg) and FcRn inhibition with efgartigimod.
- Neurological symptoms stabilized after thyroidectomy for papillary thyroid carcinoma with a BRAF V600E mutation.
- This case highlights the importance of considering autoimmune and neoplastic conditions in atypical or relapsing GBS for individualized management.
