  • Renal dysfunction was found in 29.6% of 402 symptomatic Waldenström macroglobulinaemia (WM) patients.
  • Patients with renal dysfunction were older, had lower hemoglobin levels, and higher proteinuria.
  • Renal dysfunction was associated with worse overall survival (139 vs. 203 months) and progression-free survival (80 vs. 106 months).
  • In younger patients (<70 years), renal dysfunction still correlated with shorter progression-free survival.
  • First-line chemoimmunotherapy improved median progression-free survival in patients with renal dysfunction.
  • Renal dysfunction at diagnosis may be an independent adverse prognostic factor in symptomatic WM.
