  • Programmed cell death (PCD) plays a significant role in diabetic retinopathy (DR), but genetic mechanisms are unclear.
  • Mendelian randomization (MR) was used to investigate causal relationships between PCD-related genes and DR.
  • Six genes were identified affecting DR: CTSH, NQO1, TRIB3, PGAM5 (increased risk), and IREB2, TNF (protective effects).
  • Multivariate MR confirmed significant causal effects for CTSH, IREB2, and PGAM5.
  • Pathway analysis revealed enrichment in necroptosis, apoptosis, mitophagy, and TNF signaling pathways in DR.
  • The study supports PCD's causal role in DR and identifies candidate genes for therapeutic targeting or biomarkers.
