  • Progressive exercise snacking (PES) evaluated for middle-aged and older adults with knee osteoarthritis (KOA).
  • Meta-analysis included 18 randomized controlled trials (RCTs) with 1200 participants.
  • PES improved pain, joint function, and quality of life at short-term follow-up.
  • Mid-term follow-up showed improved gait performance with PES.
  • Benefits diminished with longer follow-up periods.
  • Larger effects associated with shorter intervention duration (≤12 weeks), higher weekly exercise dose (≥300 MET·min/week), and walking as primary modality.
  • PES did not significantly increase adverse events.
  • Long-term efficacy of PES remains uncertain; more high-quality RCTs needed.
