  • Study examines the association between 24-hour systolic blood pressure (SBP) time in target range (TTR) and mortality.
  • TTR reflects the proportion of time blood pressure remains within a defined range, integrating BP variability and control.
  • Patients from the Spanish Ambulatory BP Monitoring Registry were analyzed, including those on antihypertensive medications or with untreated sustained/masked hypertension.
  • TTR was estimated using linear interpolation between consecutive SBP recordings from ambulatory BP monitoring.
  • Higher 24-hour TTR was associated with lower all-cause mortality (HR 0.83 per 1-SD increment) and cardiovascular mortality (HR 0.80 per 1-SD increment).
  • Associations remained significant after adjusting for mean 24-hour SBP and SBP variability.
  • Conclusion: Higher 24-hour SBP TTR is independently associated with lower all-cause and cardiovascular mortality.
