- The study evaluates the effectiveness of the adult recombinant zoster vaccine (Shingrix) in preventing zoster keratitis.
- A retrospective, multicenter cohort study was conducted using a global federated electronic health record network.
- Patients aged ≥50 years with no prior zoster ocular disease were included.
- Shingrix-vaccinated patients were compared with two control groups: presbyopia patients and those who received the quadrivalent influenza vaccine.
- After propensity score matching, 571,746 patients were in the presbyopia comparison and 626,697 in the influenza vaccine comparison.
- Primary outcome was incident zoster keratitis, and secondary outcome was zoster conjunctivitis over a 5-year follow-up.
- Shingrix-vaccinated patients showed a significantly reduced risk of zoster keratitis (HR, 0.50) and conjunctivitis (HR, 0.53) compared to presbyopia controls.
- Similar reductions were observed compared to influenza-vaccinated controls (HR, 0.56 for keratitis and HR, 0.37 for conjunctivitis).
- The study supports Shingrix's potential role in preventing herpes zoster keratitis.
