  • Rare case report of simultaneous chronic lymphocytic leukemia (CLL) and macrofocal multiple myeloma (MFMM) with extramedullary plasmacytoma (EMP) in a lymph node.
  • 67-year-old male presented with persistent backache, elevated white blood cell count (57.0 × 109/L), and high fluorodeoxyglucose uptake in T10, T12, and left supraclavicular lymph node.
  • Diagnosis confirmed via bone marrow aspiration and biopsy, revealing plasmacytoma and neoplastic B-lymphocytes.
  • Treatment with bortezomib, lenalidomide, and dexamethasone led to complete response, but discontinued due to grade 3 peripheral sensory neuropathy.
  • Switched to daratumumab and dexamethasone; patient remains under outpatient follow-up with 1-year follow-up duration.
