  • Primary outcome: CRRT filter thrombosis; RCA showed lower risk compared to UFH (17.0% vs 26.6% at 72h).
  • Median filter lifespan was similar between RCA (47 hours) and UFH (44 hours).
  • RCA did not increase bleeding events (33.3% vs 37.6%) or severe metabolic complications.
  • Conclusion: RCA is safe and effective, reducing thrombosis risk without additional complications in high-risk VA-ECMO patients.
