- Study aimed to develop a risk-stratification model for bladder cancer patients post-radical cystectomy (RC).
- Retrospective analysis of 3196 patients from 16 European centers (1990-2024) was conducted.
- High-Risk group identified (pT3-pT4, node-positive disease, or pT2 with lymphovascular invasion (LVI)).
- High-Risk group had significantly worse 5-year recurrence-free survival (37.8% vs 76.2%).
- Risk stratification was prognostic for recurrence, cancer-specific survival, and overall survival.
- Landmark analysis showed elevated risk persisted up to 4 years post-RC.
- Proposed risk-adapted follow-up strategy based on pathological features.
- Prospective evaluation needed to confirm clinical utility and cost-effectiveness.
