  • Advanced liver fibrosis is increasing globally, with women experiencing faster progression despite lower prevalence.
  • The study examined sex differences in the association between cardiometabolic risk factors (CMRFs) and significant liver fibrosis among US adults.
  • Data from the US National Health and Nutrition Examination Survey (2017-2020) was analyzed, including 5981 participants (2992 women and 2989 men).
  • Women had a higher prevalence of high waist circumference but lower prevalence of hypertension, glucose intolerance, and hypertriglyceridemia compared to men.
  • The prevalence of significant fibrosis was 6.9% in women and 10.7% in men.
  • High waist circumference, glucose intolerance, and the presence of 2 or more CMRFs were associated with a greater increase in risk of significant fibrosis in women compared to men.
  • The findings suggest the need for sex-specific screening approaches for liver fibrosis.
