- Stroke is a significant complication after bioprosthetic aortic valve replacement (AVR) for severe aortic stenosis (AS).
- The study aimed to determine the proportion of adults over 18 who experienced ischemic stroke post-AVR.
- A systematic review and meta-analysis included 27 studies for native AS and 5 for valve-in-valve (ViV) procedures.
- At 30 days post-TAVR, the pooled stroke proportion was 3.0%, with major and minor strokes each at 1.7%.
- At 1 year post-TAVR, the all-stroke proportion was 5.0%, with major strokes at 3.0% and minor strokes at 2.0%.
- TAVR showed significantly lower odds of stroke at 30 days compared to surgical AVR (OR 0.73).
- No significant difference in stroke rates between TAVR and surgical AVR was observed at 1, 2, or 5 years.
- For ViV procedures, the 30-day and 1-year all-stroke proportions were 2.0% and 3.0%, respectively.
- Future research should explore causes of long-term stroke, effects of antithrombotic therapies, and impact on cognitive function.
