- Sarcopenia is linked to higher mortality in liver transplant candidates, but its role in cirrhosis progression is less studied.
- The study used skeletal muscle index (SMI) from CT scans to measure sarcopenia in 1,624 cirrhosis patients across multiple continents.
- Sarcopenia independently increased the risk of disease progression or death only in decompensated cirrhosis (aHR 1.43).
- Sarcopenia also independently raised the risk of death alone specifically in decompensated cirrhosis (aHR 1.55).
- Unplanned hospitalizations were associated with sarcopenia in both compensated (aHR 1.70) and decompensated cirrhosis (aHR 1.32).
- Future research should explore whether reversing or preventing sarcopenia can slow cirrhosis progression.
