  • Ceftazidime-avibactam resistance evolution in Pseudomonas aeruginosa was studied using 12 patient isolate pairs.
  • Resistance mechanisms included amino acid substitutions in AmpC and OXA β-lactamases, and regulatory mutations causing hyper-production of AmpC and MexAB-OprM efflux pumps.
  • Cross-resistance was common with ceftolozane-tazobactam, less common with imipenem-relebactam and cefepime-zidebactam, and lowest with cefiderocol.
  • Findings have important implications for sequential treatment of P. aeruginosa infections.
