  • Lewy body pathology (LBP) prevalence was studied across nine community-based autopsy cohorts totaling 7,309 individuals.
  • Overall, 27.3% of participants had LBP, with neocortical LBP present in 8.4% and associated with dementia (OR=4.06).
  • No sex differences were found for neocortical or limbic LBP, but amygdala-predominant LBP was more frequent in women (OR=1.53) and brainstem-predominant LBP more common in men (OR=0.64).
  • Neocortical LBP was linked to Alzheimer's disease neuropathological change (ADNC) (OR=2.30) and limbic-predominant age-related TDP-43 encephalopathy neuropathologic change (LATE-NC) (OR=2.03).
  • Amygdala-predominant LBP showed strong association with ADNC co-pathology (OR=12.7), whereas brainstem-predominant LBP did not (OR=0.88).
  • Findings suggest distinct disease pathways for different LBP distributions, highlighting biological heterogeneity.
